At Tohi Psychiatry, we believe financial transparency is an important part of creating a comfortable and supportive care experience. We accept select commercial insurance plans and offer self-pay options for patients who are uninsured, have out-of-network benefits, or prefer not to use insurance.

INSURANCE

Tohi Psychiatry participates with select insurance plans. Coverage may vary based on your specific plan, location, benefits, deductible, and the services provided.

Before your first appointment, we encourage you to contact your insurance company to confirm:

• Tohi Psychiatry and Stephanie Kutella, APRN, PMHNP-BC are in network

• Your deductible, copayment, and coinsurance responsibilities

• Whether behavioral health services require authorization

• Whether telehealth services are covered

• Whether additional and third-party services are covered

Verification of benefits is not a guarantee of payment. Patients are responsible for charges not covered by their insurance plan, including applicable deductibles, copayments, coinsurance, and non-covered services.

SELF-PAY RATES

Tohi Psychiatry offers self-pay options for patients who are uninsured or choose not to use insurance. Fees vary based on the type and duration of service based on each patient’s clinical needs. We are happy to provide a detailed estimate of expected costs prior to your appointment upon request.

OUT-OF-NETWORK BENEFITS

Patients with out-of-network benefits may request a superbill to submit to their insurance company for possible reimbursement. Reimbursement is determined entirely by the patient’s insurance plan and cannot be guaranteed by Tohi Psychiatry or the provider.

PAYMENT

Payment is due at the time of service. A valid payment method must be maintained securely on file. Tohi Psychiatry accepts cash and most major credit/debit cards; please confirm prior to your appointment.

CANCELLATIONS & MISSED APPOINTMENTS

Appointments are reserved specifically for each patient. At least 24 hours’ notice is requested when canceling or rescheduling an appointment. Late cancellations and missed appointments may be charged up to the full rate of the scheduled service. These fees are generally not covered by insurance.

ADDITIONAL & THIRD PARTY SERVICES

Tohi Psychiatry and its provider/s may order laboratory or diagnostic testing when clinically appropriate. Laboratory services may be completed through Quest Diagnostics, Labcorp, or another qualified laboratory based on availability and the patient’s insurance requirements.

Laboratory testing, EKGs, and other diagnostic services are performed and billed separately by the outside facility. Patients are responsible for confirming that the selected laboratory or diagnostic facility participates with their insurance plan and for all resulting deductibles, copayments, coinsurance, noncovered services, and self-pay charges.

Fees may also apply for services completed outside of an appointment, including extensive forms, letters, record reviews, or other administrative requests. Any applicable practice fee will be discussed before the service is completed.

GOOD FAITH ESTIMATE

Patients who are uninsured or who choose not to use insurance have the right to receive a Good Faith Estimate explaining the anticipated cost of their care. Please contact Tohi Psychiatry with questions.
